We aim to help you from your first contact with us. When you first make contact with us we will send out some material about us and importantly some questionnaires, used around the world, to complete and send back, with your doctor’s referral letter. Our clinic, usually a doctor, will assess this information and we will get back to you promptly with appointments.
At Capital Pain and Rehabilitation Clinic we strive to work with you to provide the highest standard of care. To make this possible, we kindly ask that you complete the following before attending your first appointment, and bring it in with you:
It is common for your first point of contact to be a pain specialist who will guide your treatment. However, this is not always necessary before you yourself start to engage in what you may already sense is what may be helpful. This may include seeing our physical therapist, our pain management psychologist, soft tissue therapist, acupuncturist or soft tissue massage therapist. The doctor may recommend one of these to you anyway.
Depending on complexity, initial consultations may require an additional appointment to ensure a comprehensive assessment.
We aim to help you from your first contact with us, whether by phone, email or website.
We will send you some brief Questionnaires which give some additional background information to your Referral.
